This year, in January 2026, just 25% of candidates cleared FMGE. In December 2025, over 76% failed. If you are one of them, your medical career is not over. Three globally recognized licensing exams - USMLE (USA), PLAB/UKMLA (UK) and AMC (Australia) - give you a direct path to clinical practice abroad. This article tells you exactly what each exam requires and which one fits your profile.
Why Do Over 75% of Candidates Fail FMGE Every Year?
The FMGE is not a fair exam by design. The FMGE pass rate has stayed well below 30% for years. The pool of candidates keeps growing - over 44,000 sat for the December 2024 session alone - but the clearance rate barely moves.

This is not about you studying less. It is a structural bottleneck.
Here is what makes it worse: the NExT exam replacing FMGE from 2027 will be harder, not easier. The India route is tightening, not opening up.
If You Don't Clear FMGE, Is Your Medical Career Over?
No. Your MBBS degree is a globally recognised medical qualification. FMGE only controls one gate - India. It has no power over the US, the UK, or Australia.
USMLE, PLAB, and AMC are completely independent of India's NMC registration system. You do not need to clear FMGE first. You do not need to be registered in India at all.
Doctors who clear these exams and enter residency or specialty training abroad often earn more and train in better-resourced hospitals than doctors who clear FMGE and wait years for a NEET-PG seat.
What is the Alternative to FMGE for Indian Doctors in 2026?
Three structured international medical licensing exams open clinical careers in developed countries: USMLE for the United States, PLAB/UKMLA for the United Kingdom, and AMC for Australia. Each leads to full medical registration - ECFMG and NRMP residency in the US, GMC registration in the UK, and AHPRA registration in Australia.
Here is what each one involves.
USMLE - The Path to US Residency and Global Recognition
USMLE exam for Indian doctors is the most competitive path. It is also the most rewarding, financially and professionally.
- Who is eligible: Graduates from any NMC-recognised Indian or foreign medical college. Your MBBS degree must be verified through ECFMG certification.
- Exam format: Step 1 (Basic Sciences), Step 2 CK (Clinical Knowledge), Step 3 (Clinical Management), followed by participation in the NRMP residency match to secure a residency seat in the United States.
- Commitment: 2-3 years preparation for exams before taking up residency matching. Total independent practice duration is 3-4 years.
- Expenses: Exam fees, ECFMG charges and application fees cost about $8,000 to $12,000.
- After residency salary: $200,000 - $400,000 USD per year, depending on specialty.
- Best fit: Doctors who have good academic scores, research interest and you have to have financial capacity and patience for a longer runway.
PLAB / UKMLA - The Fastest Route to Clinical Practice in the UK
Historically, PLAB exam was the fastest avenue for Indian MBBS graduates to begin professional practice in any English-speaking country. The UK licensing system is now moving towards adopting the UKMLA exam 2026 format (placing the current PLAB 1 and PLAB 2 system behind us).
- Educational qualification: Indian MBBS Graduate from an NMC recognised institute. A valid IELTS or OET is also required to meet the medical licensing requirements in the UK.
- Exam Format: Part 1 is now called Applied Knowledge Test (UKMLA). The new Part 2 is Clinical and Professional Skills Assessment (CPSA).
- It takes 12-18 months for a person to be registered in Pathway to GMC registration India from the first examination.
- Total cost typically runs from £5,000 to £10,000 (including exams and initial relocation costs).
- Salary can go up to £80,000 to £120,000 annually at the specialist level.
- Who should sit this exam: Medical practitioners wishing to gain accelerated access to clinical practice and/or specialist training.
AMC - The Route to Australia and PR
AMC exam is for Indian medical graduates who aim for permanent residency in Australia. It provides an obvious way and a nice lifestyle.
- Eligibility: MBBS from any NMC approved Medical College included in the World Directory of Medical Schools (WDOMS). All students must complete an EPIC verification. Requires IELTS or OET score.
- Exam structure: AMC MCQ CAT (a computer-based test, taken globally) followed by the AMC Clinical Exam, which is held in Australia only.
- Timeline: 2 to 3 years to AHPRA registration and general practice.
- Cost: AUD 4,000 to 6,000 in exam fees. Entry-level doctor's salary is around AUD 70,000 to 90,000. GP salary can be AUD 180,000 to 300,000 or more.
- PR pathway: Good, particularly where there is interest in working in a regional or rural area.
- Appropriate for: Medical practitioners with interests in long-term settlement, work-life balance and having a clear plan for moving to Australia to practise medicine.

Do Indian MBBS Graduates need to pass the FMGE first to apply for these Exams?
No. Clearance of FMGE is not needed for USMLE, PLAB or AMC. All of these exams have no relationship to NMC registrations in India.
USMLE:
You will need to get your MBBS certified by ECFMG. No Indian registration required.
PLAB / UKMLA:
Needs a GMC-recognised MBBS and an appropriate English language test result.
AMC:
Your MBBS needs to be listed in the WDOMS. Credential verification is to be done through EPIC.
These exams can be started during internship or post MBBS.
You can begin preparing for any of these exams while still in internship or immediately after completing your MBBS.
What Non-Clinical Options Exist for FMGE-Failed Doctors in India?
In case of inability to go abroad at this moment, these posts are available for foreign medical graduates who have not appeared in FMGE.
- Roles in Clinical Research: CROs/Pharma companies do not require NMC registration.
- Hospital Administration: A degree in MHA opens doors for management and operation positions in private hospitals.
- Medical Writer: Jobs related to content, regulatory writing and pharmacovigilance in India are in demand.
- Public Health: Opportunities include MPH programs, NGO jobs and government health work.
These are real careers. But if practicing medicine is your goal, USMLE, PLAB, and AMC remain a stronger long-term investment than waiting on the FMGE cycle.
What Should You Do Next If You Have Failed FMGE?
Start with clarity, not panic. Here are the four steps that matter.
1
Assess your profile. Be honest about your budget, preferred country, timeline, and whether you lean toward clinical work or research. This one step removes two of the three exam options immediately.
2
Check your college's recognition status. Confirm your MBBS institution is listed in the WDOMS (required for AMC), and that it is NMC-recognised (required for USMLE and PLAB). Most Indian colleges qualify. Foreign university graduates should verify carefully.
3
Start English proficiency preparation now. IELTS or OET is required for all three exams. This is the one step common to every path, and it takes time. Do not wait.
4
Choose one exam and commit. Preparing for all three at once is a mistake. Pick based on your Step 1 assessment and go deep.
One final note: the NExT exam replacing FMGE from 2027 will not make the India route easier. If anything, the competition will grow and the standard will rise. The window to build an international career while your knowledge is fresh is right now.
