Does it impact IMGs or Indian MBBS graduates in any different way?

No. The process is identical for US grads and IMGs. Every USMLE examinee now uses the same FSMB portal.

What if I already paid for my transcript through MyERAS before June 4, 2026?

That old payment method no longer applies. Going forward, all payment happens through FSMB directly, not MyERAS.

Is this the same as the ECFMG-to-FSMB transition?

No. That earlier change, from January 2026, covered USMLE exam registration for IMGs. This one covers transcript delivery to ERAS, and it applies to everyone.

FSMB USMLE Transcripts for ERAS: What Changed (2026)

Since June 4, 2026, FSMB processes all USMLE transcript requests for ERAS. This applies to every USMLE examinee, whether you graduated from a US medical school or an international one. You now request and pay for your transcript directly through FSMB's USMLE portal, then retrieve it inside MyERAS. The old ERAS-specific transcript process is gone.

What Exactly Changed (Old vs New)

Before June 4, 2026, ERAS transcripts moved through their own separate track. FSMB handled other USMLE transcripts, but ERAS ran on a different system. That split is gone now. FSMB runs one process for everyone.

This is a USMLE transcript processing change, not a change to how ERAS reviews or ranks your application. Your scores, your Step history, none of that changes. Only the path your transcript takes to reach ERAS is different.

How to Submit Your USMLE Transcript Now

The process has six steps. Follow them in order, and don't skip the MyERAS step at the end.

Create or log into your account on FSMB's USMLE portal at usmle.fsmb.org.

Request a transcript and pay the fee directly through FSMB.

For USMLE Transcripts, choose "Initiate Score Retrieval."

MyERAS pulls the transcript electronically from FSMB. This usually happens the same day.

Assign the transcript to your saved or applied programs inside MyERAS.

Applying to ResidencyCAS too? OB/GYN and EM programs use it. Select both ERAS and ResidencyCAS as recipients in the same FSMB order. You won't pay twice.

Fees

The fee structure changed along with the process. Here's what it looks like now.

Check your recipients twice before you submit. FSMB will not let you add a program or fix a mistake after payment goes through. You'd have to start a new order and pay again.

Common Causes of Delay (and How to Avoid Them)

The majority of delays are caused by a few preventable errors. Keep an eye out for these.

Wrong USMLE ID in MyERAS

If the ID doesn't match, FSMB can't send your transcript to the right applicant record.

Name or date-of-birth mismatch

Small differences between your FSMB record and your MyERAS record can hold up processing.

Forgetting the second step

Paying FSMB is not enough on its own. You still have to log into MyERAS and click "Initiate Score Retrieval." These are two separate actions, and skipping the second one is the most common mistake applicants make.

Before you submit anything, open both accounts side by side. Confirm your USMLE ID, your legal name, and your date of birth match exactly. That five-minute check saves days of waiting later.

FSMB vs ECFMG: Don't Confuse These Two Changes

FSMB has made two separate changes in 2026, and applicants often mix them up.
